What to expect when you visit.

In the event that you are scheduled to be a patient with us, we have outlined some things to remember in preparation of this visit. 

You will be asked to complete a number forms for our files.  If you were referred by another physician they may have given these forms to you already.  We are happy to mail you the necessary forms if they were not provided by your referring doctor.  Having these forms completed, as well as the information outlined below, will streamline our registration process.  If you have the forms please have them filled out, in their entirety, and bring them with you to your appointment.  This will enable us to expedite the registration process and update your records. 

We will also need:

  • Your office co-payment

  • Your insurance card(s)
     
  • If your insurance company requires pre-authorization or a referral from your primary care physician, we will need you to bring this with you or fax it to 801-281-2383
     
  • Effective August 1, 2009 the Federal Trade Commission requires that we verify our patients’ identity to prevent individuals from fraudulently using insurance cards to obtain medical care.  We will require that you bring a picture ID, such as a driver’s license or passport, or a utility bill with your name and address on it, for verification purposes.   This information is required to comply with federal regulations which are intended ultimately to protect you from identity theft.

Our physicians are sub-specialists which may require a longer visit with you than you are accustomed to at many other doctors’ offices.  You should plan to be at our office for up to a couple of hours depending on whether we do diagnostic testing or not while you are here.  Please arrange your schedule to accommodate this time frame.  You are also welcomed and encouraged to have a family member accompany you to your appointment.
